6afcbc0ad5 docs: add AI disclosure checkbox to PR template 
Add checkbox to encourage contributors to disclose AI tool usage
(ChatGPT, Claude, Gemini, etc.) in their pull requests, complementing
the new AI usage policy in CONTRIBUTING.md.
